APPENDIX I: Information Requirements

1. Contact details of applicant
2. Harbour/estuary or bay the mooring is to be located within
3. GPS location of mooring
4. Draft, beam and length of vessel
5. Type of vessel (launch/yacht etc)
6. Alternative locations considered
7. An assessment of any actual or potential effects that the activity may have on the environment
8. Consultation with potentially affected local iwi, owners of legal mooring structures within 75 metres and landowners.
